General Liability Release Form
This is a standard release of liability for our massage therapy work together.
This form allows you to indicate whether you consent to the use of your photos, videos, or testimonials for marketing, social media, or educational purposes. As a thank-you for completing this form and agreeing to marketing, you will receive a warm self-heating eye mask during your first session to enhance relaxation. Your privacy and comfort are always respected, and your information is kept confidential.
This form allows a parent or legal guardian to give permission to perform massage on a minor client.
